Forge understands how important it is for breast cancer patients and survivors to get to treatment and follow up medical appointments. That’s why we offer several ways to assist, should transportation be an issue. If you need help getting to medical appointments and live or treat in Alabama, please contact our Client Services Coordinator, Janet Dees, at (205) 990-5367 or [email protected]. Si hablas español y quieres más información, por favor contacta a Yadira Robayna, al (205) 990-5375 O al [email protected].
Birmingham On Demand: If you live in the city of Birmingham, we can connect you with Birmingham On Demand, powered by Via, and get you a ride to your appointment at the flat rate of $1.50. The program serves riders Monday through Friday from 6 a.m. to 8 p.m. and Saturday from 10 a.m. to 8 p.m. They operate marked Mercedes Metris vans. The program provides accessible vehicles for riders with disabilities.
Lyft Rides: Forge has a concierge service with Lyft, through which we are able to schedule rides to medical appointments and flexible rides home that you can schedule yourself by text. Don’t have the Lyft app or a smart phone? You don’t need one. We take care of it for you.
Chevron Gas Cards: Do you live where Lyft isn’t an option or too far away from your treatment facility to use Lyft? That’s okay. Forge has Chevron gift cards we can mail you to help pay for the gas you need to get to and from your appointment. We have a formula we use to calculate the miles you need to travel and the cost of gas so that we can get you just the right amount you need.
