Survivors that come to Forge Breast Cancer Survivor Services often ask us the same questions. What do I do now? Who can help me? Where do I start? Right here. These are some of the services that Forge offers. It starts with us listening to you. Give us a call and we'll go through a brief assessment to set goals, then we'll direct you to the best Forge member, service, or resource to meet your needs.

Individualized Plans & Goal Setting

When you call Forge, one of our kind, caring, and knowledgeable staff members will work with you to go through an assessment to learn more about you and your diagnosis. We will then work with you to set goals and put together an individualized Forge Support Plan to suit your needs as you go through your journey with breast cancer. You will receive a customized resource packet containing tip sheets, resources and support; Forge Staff will follow-up with you periodically to ensure you are receiving the support you need to live your best quality of life. Advocate (Lay Navigator) Match

Forge Advocates are trained Volunteers that serve as guides for breast cancer survivors and their co-survivors by providing information, support, encouragement, navigation and assistance to help survivors and their loved ones overcome barriers. Forge Advocates can do things like go with you to your doctor’s appointment to take notes or assist with implementing certain aspects of your individualized Forge Support Plan. Peer Mentor Match

Forge’s Peer Mentor match program matches survivors and co-survivors with trained and compassionate peer mentors who provide person-centered, emotional and social support. Lifestyle Classes

Forge Gardening Classes are offered throughout the year; topics for each class tie health, wellbeing, and survivorship to a gardening topic. Participants learn a new skill & take home a creation to enjoy with family and friends. Gardening classes are for anyone who has ever been diagnosed with breast cancer, as well as their loved ones and the healthcare professionals that serve them. These classes focus on incorporating healthy exercise and lifestyle changes into the lives of breast cancer survivors and their loved ones. Survive Strong classes are for anyone who has ever been diagnosed with breast cancer, as well as their loved ones and the healthcare professionals that serve them. Forge Share and Support Groups

Forge Share and Support Groups are designed to meet the needs of breast cancer survivors in our community. Meeting topics alternate between fun and educational topics or time for sharing and supporting one another.
