Forge Survivorship Center is a holistic, patient-centered, community-based, breast cancer survivorship center that is a partnership among The Community Foundation of Greater Birmingham, Brookwood Baptist Health, Grandview Medical Center, St. Vincent’s Health System, and UAB Medicine.
